Electronic Health Records and Physician Burnout: A Scoping Review

Introduction

Electronic Health Records (EHRs) have become an essential part of modern healthcare, replacing traditional paper-based records with secure digital systems that improve accessibility, communication, and patient care. While EHRs provide numerous advantages, an increasing body of research suggests that they also contribute to physician burnout.

Physician burnout is characterized by emotional exhaustion, depersonalization, and a reduced sense of professional accomplishment. Beyond affecting physicians' well-being, burnout may negatively influence patient safety, healthcare quality, and overall clinical performance.

This scoping review explores the relationship between Electronic Health Records and physician burnout by examining the major contributing factors identified in current research.


Study Methodology

The review followed the Joanna Briggs Institute guidelines for conducting a qualitative scoping review.

Researchers initially identified approximately 500 studies, but after applying strict eligibility criteria, only 30 studies were included in the final review.

This rigorous selection process ensured that the analysis focused on high-quality evidence regarding physician burnout associated with Electronic Health Records.


Major Factors Contributing to Physician Burnout

The review identified six primary factors contributing to burnout among physicians using Electronic Health Records.


1. Documentation and Record-Keeping Responsibilities

Clinical documentation was identified as one of the most significant contributors to physician burnout.

Physicians spend substantial amounts of time completing electronic documentation, updating patient records, and fulfilling administrative requirements. These repetitive tasks often reduce the time available for direct patient interaction and increase mental fatigue.


2. Poor Electronic Health Record Design

Nine studies highlighted that poorly designed Electronic Health Record systems significantly increase physician stress.

Complicated interfaces, difficult navigation, and excessive system functionality often create unnecessary cognitive workload. Interestingly, physicians reported greater stress when using EHR systems with a large number of complex features compared with simpler, more user-friendly platforms.

The findings emphasize the importance of intuitive, user-centered EHR design to reduce frustration and improve workflow efficiency.


3. Increased Workload and Overtime

Fourteen studies found that Electronic Health Records contribute to longer working hours and increased workload.

Many physicians reported spending additional time during evenings and weekends completing documentation, reviewing records, and fulfilling EHR-related responsibilities. This extended workload disrupts work-life balance and increases the risk of professional burnout.


4. Indirect Patient Care Activities

Eight studies examined burnout associated with indirect patient care responsibilities performed through Electronic Health Records.

Tasks such as:

  • Clinical documentation
  • Electronic communication
  • Medical note preparation
  • Administrative record updates

consume considerable physician time and contribute to increased workplace stress.


5. Alert Fatigue and Inbox Notifications

Six studies identified alert fatigue as another major contributor to physician burnout.

Electronic Health Records generate continuous notifications, including:

  • Laboratory results
  • Medication refill requests
  • Clinical reminders
  • Patient messages

The constant stream of alerts requires immediate attention, increasing interruptions and reducing physicians' ability to focus on patient care. Effective alert management strategies are therefore essential to reduce unnecessary cognitive burden.


6. Consequences of EHR-Related Burnout

The review also examined the broader effects of burnout associated with Electronic Health Records.

Potential consequences include:

  • Reduced quality of patient care.
  • Professional misconduct.
  • Mental health concerns.
  • Lower patient satisfaction.
  • Increased risks to patient safety.

These findings demonstrate that physician burnout affects not only healthcare professionals but also the overall quality of healthcare delivery.


Discussion

The review supports findings from previous research showing that physicians spend a considerable portion of their working day interacting with computers rather than patients.

A growing phenomenon known as "doctoring while typing" illustrates how physicians often divide their attention between patient communication and electronic documentation. This contributes to longer documentation times, information overload, and increased cognitive stress.

The review also emphasizes that alert fatigue remains a significant challenge and recommends collaborative strategies to improve notification management and reduce unnecessary interruptions.


Summary

This scoping review demonstrates that although Electronic Health Records have transformed healthcare delivery, they also introduce several workplace challenges that contribute to physician burnout. Documentation requirements, poor system design, excessive workload, indirect patient care activities, and alert fatigue were consistently identified as major contributors. Addressing these issues through improved system usability, workflow optimization, and better support strategies can help reduce physician stress while maintaining the benefits of digital healthcare.


Conclusion

Electronic Health Records remain a vital component of modern healthcare, but their implementation should prioritize both clinical efficiency and physician well-being. The findings of this review highlight the importance of designing user-friendly systems, reducing unnecessary administrative burden, improving alert management, and providing ongoing support for healthcare professionals. Future research should continue exploring effective strategies to minimize burnout among physicians and other healthcare providers who rely on Electronic Health Records in their daily practice.
